At home nurse

The home nurse provides nursing care to persons, usually dependent or suffering a temporary loss of autonomy due to alterations in their general health, who require healthcare at home on a regular basis. Their activities focus on the promotion, protection and rehabilitiation of the health of patients with chronic or terminal pathologies, with dementias which require transitory nursing care over extended periods of time, or who due to their physical characteristics or that of their environment (isolation, age, loss of autonomy) are unable to travel to a healthcare centre.

• Prepare the necessary materials and process the information required to operate in his/her areas of responsibility.
  Collect and record the details of patients or customers.
  Check that the necessary equipment is available to carry out the tasks involved in each consultation.
  Collect and issue the verbal and written information needed to achieve a good coordination with other professionals.
  Draw up quotes and invoices for healthcare centres. Prepare the medical consultation, if applicable.
• Provide auxillary nursing care to patients / clients.
  In charge of the personal care of patients, washing and dressing them according to their needs and the care plan.
  If applicable, prepare patients to be moved and ensure that the necessary conditions are fulfilled.
  Move and transport patients in their surroundings, taking into account their condition and the recommended anatomical position.
  Help patients move around.
  Carry out the necessary operations to ensure the patient/client can be examined and observed as need be.
  Apply topical medical thermotherapy, cryotherapy and hydrology treatments.
  Prepare patient medication.
  Carry out post-mortem care according to established protocols.
  Distribute meals and offer patients to help them eat if they like.
  Participate in providing first aid or accident prevention services.
• Provide healthcare (non auxiliary) and patient/customer follow-up in own areas of competence.
  Examine, assess, identify and carry out a nursing care diagnosis as a preliminary service which complements those provided by other professionals.
  Administer medication by oral, rectal or topical route.
• Ensure optimal conditions of sanitisation of the healthcare equipment used on the patient and in the patient 's surroundings.
  Clean, disinfect and sterilise the material and instruments used in the different consultations.
  Keep the patient's home clean, tidy and hygienic.
  Participate in infection control.
  Monitor and maintain the instruments and machines used during patient care.
• Contribute to providing psychiatric care to patients/customers, using psychological support techniques and healthcare education.
  Identify basic psychological needs and any abnormal behaviour in the different kinds of patients.
  Provide basic psychological support to patients / clients to improve their quality of life.
  Promote self-care attitudes and habits among patients / clients.
  Intervene in educating patients/customer and their relatives, promoting healthy nutrition, personal hygiene and environmental & lifestyle habits.
